I am based in Depew, New York, which means US business hours, a US telephone number, and a certificate issued by a business registered in New York State. For an immigration filing that matters more than it sounds: if a caseworker or an attorney has a question about the translation, there is somebody in the same time zone to answer it.
Quoted per document rather than per word, because a one page birth certificate and a twelve page police record are not the same job. Send a photograph or a scan and you will have a figure back, usually the same business day.

No. The regulation asks for a full English translation, certified by the translator as complete and accurate, together with the translator's statement that they are competent to translate. No accreditation is required of anyone.
Worth knowing, because it cuts both ways: the ATA offers no certification exam in Hindi, Urdu, Marathi or Gujarati at all, so nobody working in these languages can hold that credential. Active ATA membership, which I do hold, is the ceiling. Anyone advertising themselves as an ATA certified Hindi or Urdu translator is describing something that does not exist.
USCIS does not require notarisation of a translation. Some universities, courts and foreign consulates do. Tell me if yours does and it can be arranged.
If a translation I certified is questioned on accuracy or completeness, I correct it and reissue the certificate at no charge. That is what the certificate is for.
Usually. Faded registers, older handwriting and regional spellings of names are ordinary in this work. If something genuinely cannot be made out, it is marked illegible rather than invented.
